Welcome to the "The Systems View on HIV/AIDS: A Health Systems 20/20 Blog". This forum was launched on the occasion of the XVIIth International AIDS Conference to stimulate discussion and debate on how best to bring a health systems approach to the HIV/AIDS topic area; going beyond what Ambassador Dybul has called the "sterile" debate of vertical vs. horizontal programming to take in a view of the entire health system. With such a view, it is possible that we will conduct planning and implementation such that the global HIV/AIDS effort strengthens the systems for quality interventions in prevention, care, and treatment - not just for HIV/AIDS, but for all the diseases and conditions that cause morbidity and mortality in the developing world.

We hope the blog will stimulate lots of interesting conversations about specific examples of health system strengthening approaches in the HIV/AIDS world that have improved health outcomes. In the coming weeks, we will explore some of the topics from the presentations by HealthSystems 20/20 staff at the International AIDS Conference in Mexico City: human resource mobilization in PEPFAR countries such as Nigeria, service provision, and the private sector's crucial role in supplementing the the public provision of HIV/AIDS services. Interspersed with these topics, we hope to bring you news and analysis of new theoretical approaches, and insights from our in-house experts. One of these insights follows in our introductory post from Dr. Hong Wang, an international health financing expert - on the issue of 'diagonality' in HIV/AIDS programming.

Congratulations to the Health Systems 20:20 team for bringing out this excellent and informative blog.

Please find below details about a discussion on HIV and Health System Strengthening, currently going on in the UN Solution Exchabge AIDS Community of Practice in India. Though it is a vast topic, the query poser has requested three specific aspects namely,

* Areas of Health Care Systems that have been strengthened as a direct result of the responding to HIV

* Examples from the developing world of the areas in health care delivery, where success or lessons learnt from HIV have been transferred and applied

* Impact of HIV epidemic response on the health care delivery in the developed world and ways in which the developing countries can learn from them


Based on your areas of expertise, I request you to contribute to the discussion focussing on how the response to HIV helped (or can help) in strengthening (or worsening) Health Systems.

The global HIV/AIDS effort as well as stakeholders response in developing world, no doubt, strengthens the interventions quality but there is lot to be done in countries with poor resource settings. In India, USAID/India Country Strategic Plan, the Government of India’s National AIDS Control Organization (NACO) Guidelines, NACP III Guidelines for Targeted Interventions, support implementation of the US Government’s President’s Emergency Plan for AIDS Relief (PEPFAR) in India. Such initiatives are welomed.
